- A tooth is so severely damaged or decayed that an effective restoration (such as a filling, inlay, onlay, or crown) is not possible.
- Serious infection has destroyed a large portion of the tooth and supporting bone structure.
- There is not enough room for all of the teeth to properly fit in the mouth, resulting in severe crowding that cannot be corrected through orthodontic treatment alone.
- A baby (primary) tooth is not falling out as usual and is blocking the way for an erupting permanent tooth.
